Patient online forms - eForms
Micro Macro IT solutions developed an eForms module which integrates to Profile to simplify practice paperwork.
Our approach was to develop an eForms engine which allows patients to fill in practice paperwork electronically and submit that electronic document securely to the practice’s Profile system:
Patients can fill out practice forms online
These forms can be customised to each individual practice needs
Forms can be for new patient registrations, updating existing patient details, gaining consent (complete with signatures)
Forms can also be used to capture clinical information for your specific specialty
Patients can fill out these forms on their own devices at home (computers, tablets, phones) or use a practice tablet in the waiting room
Once a submission is made that document is then imported securely into profile against the patient record as a PDF in the documents area of Profile
If the patient is a new patient, the electronic information can be used to create a patient in Profile.
Existing patients can have their details updated when importing the eForm
This process securely manages the gathering of patient details before their consult and gets it into your practice with minimum double handling and data entry
Information can be used to drive other clinical processes like creating encounters, creating tasks so the eForms engine is very adaptable
